Remember to include your
name, IC number, state and
telephone number.
Do give us (the MUET secretariat) a call at 03-2773 2000 to verify that we have received the fax and all the important information can be seen clearly. If not, we will request that you resend the fax again.
Alternatively, you may scan / screen-capture the confirmation of payment and email it to
